Big Hearts Day Habilitation Center Client Application Fee Policy

Effective Date: June 17, 2024

Purpose

This policy outlines the requirement and process for a $100.00 client enrollment fee at Big Hearts Day Habilitation Center to secure enrollment and services.

Policy

  1. Enrollment Fee Requirement:
    • A non-refundable enrollment fee of $100.00 is required to secure a place for each new client at Big Hearts Day Habilitation Center.
    • This enrollment fee must be paid at the time of enrollment to confirm the client’s spot.
  2. Payment Methods:
    • The enrollment fee can be paid via check, money order, cashier’s check, debit card or credit card. Checks should be made payable to Big Hearts Day Habilitation Center.
    • Payments can be made in person at the center or through our online payment system.
  3. Enrollment Fee Application:
    • The enrollment fee will be applied toward the administrative fee.
  4. Refund Policy:
    • The $100.00 enrollment fee is non-refundable. This policy applies regardless of the reason for withdrawal from the program.
    • Exceptions to this policy will be considered on a case-by-case basis, at the discretion of the center’s management.
  5. Enrollment Confirmation:
    • Enrollment is not confirmed until the enrollment fee is received. The center will issue a confirmation of enrollment once the enrollment fee is processed.
  6. Failure to Pay Enrollment Fee:
    • If the enrollment fee is not received within the specified enrollment period, the client’s spot may be offered to another individual on the waiting list.
  7. Additional Fees:
    • Any additional fees or costs associated with services beyond the initial enrollment fee and monthly service fees will be communicated to the client and must be paid as per the center’s billing policies.

 

Procedure

  1. Submission:
    • Clients or their guardians should submit the enrollment fee along with the completed enrollment forms.
    • For online payments, a receipt of the transaction should be provided to the center.
  2. Confirmation:
    • The center will confirm receipt of the enrollment fee and enrollment through email or a physical confirmation letter.
  3. Record Keeping:
    • The enrollment fee transaction will be recorded in the client’s file and the center’s financial records.
