Our application process is designed to see our program in action, meet our staff, and discuss the goals of each prospective client.
To begin the application process:
- Call and schedule a tour or sign up for a tour on our website: 682-419-1914.
- Interview with prospective Clients, parents/guardians, and the Big Hearts Admissions team.
- Complete the client application fee policy and the client application, save them as a PDF file, and email them to info@BigHeartsDayHab.org.
- Submit the application fee of $100.00 (cashier's check, personal check, or money order). Please write “Application Fee” and the prospective Clients name in the Notes section.
- Complete the Health Assessment/Individual Service Plan Policy, save it as a PDF file, and email it to info@BigHeartsDayHab.org.
In addition to the completion of the application, please submit the following to info@BigHeartsDayHabilitationCenter.org:
- Guardianship paperwork (if applicable).
- Doctor’s orders for medication/treatment (if applicable).
- Most recent copy of the following:
- Full Individual Evaluation (FIE) (if applicable)
- Individual Education Plan (IEP) (if applicable)
- Behavior Modification Plan (if applicable)
- Any evaluations completed by Metro Care, and your provider. (if applicable)
- Respite is provided on Saturday and Sunday. We are open from 6:00 am to 4:00 pm. Please complete the respite application form, save it as a PDF file, and email it to info@BigHeartsDayHab.org.
*Big Hearts accommodates private pay clients.
*The level of need is determined by the client's provider or the Executive Director upon review of the prospective Client's application, interview, and the client's official eligibility.
Admission
- Age of 18 years or older (Graduated from high school)
- Physically or cognitively impaired
- Ambulatory (or mobile with assistive devices)
- Able to participate and behave in a socially accepted manner
- Not bedridden
- Not harmful to self or others

- July 4th
- Labor Day
- Columbus Day
- Veterans Day
- Thanksgiving Day and the day after
- Christmas Eve
- Christmas Day
